Organic Acids Test (OAT) UK - Comprehensive Urine Metabolic Testing from Mosaic Labs
The Organic Acids Test from Mosaic Laboratory is a comprehensive metabolic test that analyses 76 urinary biomarkers in a single first-morning urine sample. This functional medicine urine test evaluates intestinal microbial overgrowth, vitamin and antioxidant status, neurotransmitter metabolites, fatty acid metabolism, oxalate levels, mitochondrial function and detoxification capacity. It is the only OAT that includes markers for oxalates, which are associated with a range of chronic conditions. The test is suitable from age two and can be ordered as an at-home urine test kit.
Practitioners use the organic acids test to investigate a wide range of clinical presentations. It is commonly requested for people experiencing chronic fatigue, brain fog, digestive disturbances or unexplained mood changes, including anxiety and depression. Because the test identifies yeast overgrowth, Clostridia-related bacterial imbalances and nutrient deficiencies that conventional blood work may miss, it is also valuable for those with gut dysbiosis, neurotransmitter imbalances, hyperactivity, behaviour disorders and movement disorders. Elevated oxalate markers can help explain symptoms linked to fibromyalgia, kidney stones and chronic pain, while mitochondrial markers shed light on persistent low energy and poor metabolic health. For anyone seeking a metabolic health screening that goes beyond standard testing, the OAT provides a detailed starting point for personalised treatment planning.

Q. What's the process?
Your kit is posted to you from the UK within 24-48 hours of ordering so you should receive your test kit within a few days via DHL. You can then arrange for DHL to collect your sample using the free returns service. If you are ordering from outside the UK we will ship via DHL or Fedex with return labels.
The kit comes with full instructions on urine collection and storage. We recommend you take your sample first thing in the morning. The sample comes with a freezer bag. Please freeze your sample for a minimum of 4 hours before you arrange for DHL to collect.
When we receive your results back from Mosaic Laboratory in the USA (approx 2 weeks) we will email you your PDF report along with our functional medicine advice to support your recovery depending on the support option you selected at checkout.
Q. How much urine is required?
Specimen requirements are 10 mL of your first morning urine before food or drink is consumed.
Q. I go to the toilet several times in the night - will this make a difference?
The reason we ask for your first morning urine is that it is normally the most concentrated sample and has the highest PPM (parts per million). If you have bladder issues, we suggest picking another time in the day, but the minimum requirement for an accurate result is 3 hours between urination.
Q. Do I need to take any supplements or change my diet before taking the test?
Avoid apples, grapes, pears, and cranberries as well as their juices or products 48 hours before collection. Avoid arabinogalactan, echinacea, reishi mushrooms, and ribose supplements for 48 hours before collection. The reason we use Mosaic Labs is for the high levels of sensitivity of their equipment. When you order testing through our website you will be asked to complete a form highlighting which supplements and medications you take so our functional medicine team know what your current protocol is and can advise or adjust as required
Q. How will I know if my sample is good enough for an accurate reading?
Mosaic are the best labs in the world for this type of testing. They will immediately reject any samples not dense enough for testing and let us know. If this is the case, we will send you another test kit.
Q. Is there an age limit for this testing?
No, although we recommend not starting testing on children under 2. If the patient is young or incontinent, then you may need to manually retrieve a sample via a potty or bedpan.
Q. What does the Organic Acids Test measure?
The OAT analyses 76 urinary metabolites grouped across several clinical categories — intestinal microbial markers (yeast and bacterial overgrowth), oxalate metabolism, glycolytic cycle and mitochondrial markers, neurotransmitter metabolites, nutritional markers (including B vitamins, vitamin C, CoQ10 and biotin), fatty acid and amino acid metabolism, detoxification indicators, and phosphoric acid and creatinine. Q. Who should consider taking the Organic Acids Test?
The OAT is one of the most broadly useful tests in functional medicine because it provides a metabolic snapshot across multiple body systems simultaneously. You may want to consider it if you are experiencing any of the following:
- Chronic fatigue, persistent low energy or unexplained lethargy
- Brain fog, poor concentration or memory difficulties
- Digestive issues such as bloating, irregular bowel movements or suspected gut dysbiosis
- Recurrent yeast infections or suspected Candida overgrowth
- Mood imbalances including anxiety, depression or mood swings
- Behavioural or neurological conditions such as hyperactivity, movement disorders or Autism Spectrum Disorders
- Chronic pain, fibromyalgia or suspected oxalate-related symptoms including kidney stones and vulvodynia
- Suspected nutritional deficiencies despite a varied diet
- Migraines or recurrent headaches without a clear cause
- A history of frequent antibiotic use that may have disrupted gut flora
- Blood sugar instability or difficulty managing weight
The test is suitable from age two, making it a valuable option for children as well as adults.
Q. How does the OAT compare to a standard NHS blood test?
Standard NHS blood tests typically assess a limited set of individual markers — for example, a full blood count, thyroid function or basic vitamin levels. The Organic Acids Test takes a different approach by measuring 76 metabolites in urine, which reflect how your body is actually processing nutrients, producing energy, managing neurotransmitters and handling microbial byproducts. This means the OAT can identify functional imbalances — such as early-stage nutrient depletion, subclinical mitochondrial dysfunction, bacterial or yeast overgrowth, and neurotransmitter disruption — that may not yet show as abnormal on a standard blood panel. It is particularly useful when conventional results come back as "normal" but symptoms persist, because it looks at the metabolic pathways behind those symptoms rather than isolated blood values.
Q. Can the OAT detect yeast overgrowth and harmful gut bacteria?
Yes. The OAT includes specific microbial markers that detect byproducts of intestinal yeast (including Candida albicans) and harmful bacteria such as Clostridia species. For example, Arabinose is a key marker for yeast overgrowth, while HPHPA (3-(3-hydroxyphenyl)-3-hydroxypropionic acid) indicates Clostridia activity, and 4-Cresol is associated with C. difficile. The test also measures DHPPA, a marker for beneficial bacterial activity, so your results show the balance between harmful and helpful organisms. This makes the OAT a practical gut dysbiosis test for anyone experiencing digestive symptoms, sugar cravings, fatigue or mood disturbances that may be linked to microbial imbalance. If gut infections are a primary concern, your practitioner may also recommend a comprehensive stool test such as the GI360 Test to provide a complementary view of your digestive health.
Q. What can the OAT reveal about neurotransmitter and mood health?
The test measures metabolites of key neurotransmitters including dopamine (via Homovanillic Acid / HVA) and norepinephrine (via Vanillmandelic Acid / VMA), as well as the HVA:VMA ratio which can indicate disruptions in brain chemistry — sometimes linked to Clostridia overgrowth in the gut. It also measures Quinolinic acid, an inflammatory marker that, when elevated, can damage nerve cells and contribute to neurological symptoms. Additionally, 5-Hydroxyindolacetate (5HIAA) reflects serotonin metabolism, and Kynurenic acid relates to the tryptophan pathway, which influences both mood and inflammation. These markers make the OAT a useful neurotransmitter imbalance test for people experiencing anxiety, depression, mood swings, poor concentration or brain fog — particularly when standard mental health assessments have not identified a clear cause.
Q. Can I order the Organic Acids Test from outside the UK?
Yes. The test kit ships worldwide. For most countries you can simply select your location at checkout and the kit will be dispatched via DHL or FedEx with return labels included. However, if you are based in Australia, Bahrain, Bermuda, Canada, Hong Kong, India, New Zealand, Oman, Qatar, Saudi Arabia, Singapore, South Africa, the United Arab Emirates or the USA, you will need to contact the clinic at info@mybodyfabulous.co.uk for a shipping quote before ordering. Your urine sample is returned to Mosaic Laboratory in the USA using the prepaid shipping label provided with the kit.
Q. What do oxalate markers on the OAT mean for my health?
The OAT is the only organic acids test that includes markers for oxalate metabolism, specifically Oxalic acid and Glycolic acid. Elevated oxalates can result from dietary intake, gut microbial imbalance, genetic factors or nutrient deficiencies (particularly vitamin B6). High oxalate levels are associated with kidney stone formation, but oxalate crystals can also deposit in joints, muscles and other tissues, contributing to chronic pain, fibromyalgia and vulvodynia (vulvar pain). If your OAT results show raised oxalates, a practitioner can advise on dietary modifications, targeted supplementation and whether further investigation — such as the Hepatic Detox Profile or a Detoxification DNA test — would help clarify your detoxification capacity.
Q. How does the OAT relate to other tests offered by the clinic?
The OAT provides a broad metabolic overview, and depending on your results, your practitioner may recommend complementary testing to investigate specific findings in more depth:
- If microbial markers suggest yeast or bacterial overgrowth, a comprehensive stool test can provide species-level detail on gut infections and digestive function.
- If oxalate or detoxification markers are elevated, the Hepatic Detox Profile assesses liver detoxification pathways via a urinary challenge test.
- If you suspect environmental toxin exposure alongside abnormal OAT results, the Mycotoxin Mould Test or the Glyphosate Test (both also from Mosaic Labs) can identify specific toxins.
- If nutritional or detoxification markers are persistently abnormal, a Detoxification DNA test or Minerals & Metals DNA test can reveal genetic factors affecting how your body processes nutrients and clears toxins.
Many clients order the OAT as a first-line screening and then add targeted tests based on what the results highlight.
Q. Is the OAT useful for children with behavioural or neurological conditions?
Yes. The OAT is widely used in functional medicine for children with Autism Spectrum Disorders, hyperactivity, behaviour disorders and movement disorders. It can identify microbial imbalances, nutrient deficiencies and neurotransmitter disruptions that may contribute to these presentations. The test is suitable from age two. For younger or incontinent children, a sample can be collected via a potty or bedpan. Because the test only requires a 10 mL urine sample — with no blood draw — it is generally well tolerated by children.
