MensKey and UroKey Tests UK - Advanced Semen and Urine Microbial Screening by MicroGenDx
The MensKey and UroKey tests from MicroGenDx use qPCR and next-generation sequencing (NGS) to screen for bacterial, fungal, and pathogenic organisms in male urine and semen samples - detecting species that standard laboratory cultures routinely miss. The qPCR panel quantifies total bacterial load alongside 11 named pathogens and 11 antimicrobial resistance genes, while the NGS component compares extracted DNA against a reference library of over 50,000 microbial species to identify exactly what is present. UroKey analyses urine only and is suited to isolated bladder or lower urinary tract concerns; MensKey Complete adds a semen sample for cases where infection may involve the prostate, seminal vesicles, or wider reproductive tract. Both tests are available to order in the UK and ship worldwide.
Practitioners recommend this men's microbial screening when a patient presents with chronic prostatitis, recurrent UTIs that have not resolved with standard antibiotic courses, epididymitis, post-ejaculatory pain, or persistent pelvic discomfort without a clear diagnosis. It is also valuable where previous urine cultures have returned negative despite ongoing urinary symptoms, or where biofilm-protected infections in the reproductive tract are suspected. Because the report includes antimicrobial resistance gene data, clinicians can move beyond empirical prescribing and select targeted treatment strategies — making these tests particularly useful for men with a history of repeated antibiotic therapy who have not seen lasting improvement.
Which Test Should I Order?
Prostate or seminal vesicle pathogens may not appear in urine, especially if infection is localised or intracellular. Biofilms and low-level infections in reproductive tissues can cause persistent symptoms but yield negative urine cultures. When infection could originate beyond the bladder (e.g., chronic prostatitis, pelvic pain, reproductive tract involvement), we advise the MensKey Urine & Semen screening. The UroKey urine only screen suffices for isolated bladder or lower urinary tract issues.

Q. When should you consider this test?
Consider MensKey Complete for precise diagnostics of chronic UTIs, Prostatitis, and Epididymitis. It detects bacterial load, pathogens, and resistance genes via qPCR and NGS, aiding treatment decisions in complex cases. Ideal for unresolved symptoms and recurrent infections.
Consider UroKey for comprehensive diagnostics of chronic UTIs. It detects bacterial load, pathogens, and resistance genes via qPCR and NGS, aiding treatment decisions in cases of unresolved symptoms, recurrent infections, negative culture results, and prior antibiotic therapy. Ideal for guiding targeted therapies.
Q. What is the process?
Your kit is posted to you from the UK within 24-48 hours of ordering so you should receive your test kit within a few days via DHL. You can then arrange for DHL to collect your samples using the free returns service. If you are ordering from outside the UK we will ship via DHL or Fedex with free return labels.
The urine sample MUST be collected BEFORE the semen sample. Please complete both requisition forms
Q. What happens to my sample after testing?
All samples are destroyed after 6 months. Your DNA or data is never shared or sold. We use secure, encrypted servers and anonymised barcoding to ensure your privacy and security at every step.
Q. What organisms and resistance genes does this test screen for?
Both the MensKey Complete and UroKey tests use a qPCR panel to detect and quantify the following organisms and markers:
- Total 16S Bacterial Load
- Mycoplasma hominis
- Enterococcus faecalis
- Streptococcus agalactiae
- Klebsiella pneumoniae
- E coli
- Gardnerella vaginalis
- Mobiluncus curtisii
- Mobiluncus mulieris
- Prevotella bivia
- Ureaplasma parvum
- Candida albicans
The qPCR panel also screens for 11 antimicrobial resistance genes covering quinolone, methicillin, vancomycin, beta-lactam, carbapenem, macrolide, aminoglycoside, tetracycline, Bactrim, and extended spectrum beta-lactamase (CTX-M) resistance. In addition, the NGS (next-generation sequencing) report compares your sample DNA against a library of over 50,000 microbial species to identify organisms beyond the targeted qPCR list.
Q. How does this test differ from a standard NHS urine culture?
A standard NHS urine culture grows bacteria on a plate under set conditions and typically identifies only the most dominant, fast-growing organisms — often just E coli. If the infection involves slower-growing species, anaerobic bacteria, fungi such as Candida albicans, or organisms protected within biofilms, a conventional culture may return a negative result even though the patient still has symptoms. The MensKey and UroKey tests bypass this limitation entirely by using molecular methods. qPCR detects bacterial DNA directly — including from dead or non-culturable organisms — and quantifies the load present, while NGS sequences all microbial DNA in the sample against a reference database of 50,000+ species. The result is a far more complete picture of what is actually in the urinary or reproductive tract, along with antimicrobial resistance gene data that standard cultures do not provide.
Q. Should I choose UroKey (urine only) or MensKey Complete (urine and semen)?
The choice depends on where your symptoms are located and what your clinician suspects. UroKey is appropriate when the issue appears to be confined to the bladder or lower urinary tract — for example, recurrent UTIs with clear urinary symptoms but no reproductive or pelvic involvement. MensKey Complete is recommended when infection could originate beyond the bladder, particularly in cases of chronic prostatitis, pelvic pain, post-ejaculatory discomfort, or epididymitis. Pathogens in the prostate or seminal vesicles may not shed into urine in detectable quantities, especially if the infection is localised, intracellular, or protected by biofilm. Adding the semen sample significantly increases the chance of identifying the causative organism in these more complex presentations.
Q. How do I collect my urine and semen samples at home?
Full collection instructions are included with your kit, and printable PDFs are available on the product page above. The key points to be aware of are:
- The urine sample must be collected before the semen sample — this order is important for accurate results
- Both samples require a completed requisition form, which is included in the kit
- Once collected, samples are returned via DHL using the free returns service provided with your kit
Detailed step-by-step preparation and collection guidance — including hygiene steps, timing, and how to seal and package your samples — can be found in the MensKey Complete instructions or UroKey instructions.
Q. What do the antimicrobial resistance genes in my report mean?
The qPCR panel screens for genes that confer resistance to major antibiotic classes. If a resistance gene is detected, it means organisms in your sample carry the genetic machinery to survive treatment with that antibiotic group — for example, a positive quinolone resistance result suggests that fluoroquinolone antibiotics such as ciprofloxacin may be less effective. The report covers resistance to quinolones, methicillin, vancomycin, beta-lactams, carbapenems, macrolides, aminoglycosides, tetracyclines, Bactrim (trimethoprim-sulfamethoxazole), and extended spectrum beta-lactamases (CTX-M). This information allows your prescribing clinician to avoid antibiotics that are unlikely to work and choose a targeted treatment strategy, which is especially important for men who have already been through multiple courses of broad-spectrum antibiotics without lasting resolution.
Q. Can I order this test from outside the United Kingdom?
Yes — this test ships worldwide. Kits are dispatched from the UK within 24–48 hours of ordering via DHL or FedEx, and a free return label is included for sending your samples back to the laboratory. For most countries you can simply select your location at checkout. However, if you are based in Australia, Bahrain, Bermuda, Canada, Hong Kong, India, New Zealand, Oman, Qatar, Saudi Arabia, Singapore, South Africa, the United Arab Emirates, or the USA, you will need to contact the clinic for a shipping quote before placing your order.
Q. Is this test suitable for children or teenagers?
The UroKey urine-only test is suitable from age 2 years, making it an option for investigating recurrent urinary infections in boys and young males where standard cultures have not provided answers. The semen component of MensKey Complete is suitable from age 18 years only. If you are considering this test for a child or adolescent, the urine-only option is the appropriate choice.
Q. How does this test relate to other men's health tests offered by Body Fabulous?
The MensKey and UroKey tests focus specifically on identifying microbial infections and resistance patterns in the urinary and reproductive tract. Depending on your symptoms and results, your practitioner may recommend additional testing to build a fuller clinical picture:
- If prostate health is a concern — a PSA finger-prick blood test can screen for elevated prostate-specific antigen alongside your microbial results
- If hormonal symptoms such as low libido, fatigue, or erectile dysfunction are also present — a testosterone saliva test or the DUTCH Complete hormone panel can assess whether hormonal imbalances are contributing
- If you are investigating male fertility — the Male Fertility DNA Test examines genetic factors affecting sperm quality and reproductive function, complementing the infection data from MensKey
Your practitioner can advise on which combination of tests is most appropriate based on the clinical picture at your consultation.
Q. Can this test detect sexually transmitted infections?
The MensKey and UroKey tests are not designed as a dedicated STI screening panel, but the NGS component sequences all microbial DNA present in your sample against a database of over 50,000 species. This means that if sexually transmitted organisms are present in your urine or semen at the time of testing, they may be identified in the report. The qPCR panel also targets specific organisms such as Mycoplasma hominis and Ureaplasma parvum, which can be sexually transmitted and are associated with urethritis and reproductive tract inflammation. However, if your primary concern is STI screening, you should discuss this with your practitioner to determine whether a dedicated STI panel would be more appropriate alongside or instead of this test.
