Biological Age Test UK - Epigenetic DNA Methylation Test for Ageing, Longevity and Disease Risk

The Epigenetic Biological Age Test (TruAge Complete) is a finger-prick blood spot test that measures how fast your body is ageing at a cellular level by analysing DNA methylation patterns across more than 850,000 CpG sites. Developed in partnership with Harvard University and Zymo Research, the report delivers a biological age score (OMICmAge), a pace-of-ageing speedometer (DunedinPACE), individual age scores for 11 organ systems (SYMPHONYAge), telomere length, a 12-cell immunity profile, inflammation scores based on CRP and IL-6, and risk assessments for mortality, cancer, grip strength, gait speed, and the impact of smoking and alcohol. Unlike a standard DNA test that reads your fixed genetic code, this epigenetic test captures the methylation changes that occur in response to how you live - making it a dynamic, repeatable measure that can track whether your diet, exercise, sleep, stress management, and supplement protocols are genuinely slowing your biological clock.

This biological age test is used by practitioners and longevity-focused patients who want an objective, science-backed measurement of how their body is handling the ageing process - and, crucially, whether interventions are working. A higher biological age relative to chronological age is associated with an increased risk of age-related diseases including Alzheimer's disease and certain cancers, while a lower biological age correlates with reduced disease risk and a longer lifespan. Because approximately 60 per cent of DNA methylation is influenced by changeable factors - diet, exercise, sleep quality, stress, and environmental exposures - the test gives patients and practitioners a powerful baseline from which to design and monitor anti-ageing, preventive health, and longevity strategies over time.

Test is suitable from age 18 years and over.

Q. How do I take this test?

Please REGISTER YOUR KIT before sending your sample by using the QR code or go to www.trudiagnostic.com/register. The test is a simple, finger prick blood spot test. Keep your blood spot card in the fridge until you are ready to ship back. Ideally arrange collection same day.

Q. Should I have any other tests alongside the epigenetic age DNA test?

If possible, we recommend you do have a full blood test to check your current vitamin and mineral levels to optimise your DNA testing. We can offer a private full blood test – this is available in the UK only and involves having a nurse come to your home / hotel to take your sample. Patients must be over 18 years old for this test. Results are provided separately to your DNA report.

We offer a huge range of testing so when a member of our genetics team is assigned to review your results they may feel you would benefit from other specific testing such as methylation DNA, hormones (testosterone / estrogen, cortisol levels etc), microbiome, food intolerance or mould / toxin testing. If any additional testing is required, this will be advised in your written report.

*Note: if you are adding multiple tests to your basket, you only need to complete the patient form once.

Q. When should testing not be conducted?

Transient stress, such as a viral infection, can temporarily increase biological ageing. The lab does not recommend sample collection for individuals who have had a recent acute infection, surgery, trauma, or pregnancy.

A person should wait until they have fully recovered from the infection before taking a biological age test. The exact duration of recovery may vary depending on the severity and type of infection, so it is advisable to consult with the healthcare practitioner in charge for guidance.

Q. What does the TruAge Complete report include?

The report provides a comprehensive picture of biological ageing across multiple dimensions:

  • Biological Age Score (OMICmAge) — your overall biological age calculated from DNA methylation patterns, compared against your chronological age
  • 11 Organ Systems Age Scores (SYMPHONYAge) — individual ageing scores for major organ systems, showing which parts of your body are ageing faster or slower than expected
  • Pace of Ageing (DunedinPACE) — a real-time speedometer that measures how quickly you are currently ageing, expressed as years of biological ageing per calendar year
  • Telomere Length Report — an assessment of the protective caps on the ends of your chromosomes, which shorten with age and cellular stress
  • 12 Cell Immunity Report — a breakdown of immune cell populations to evaluate immune system ageing
  • Inflammation Score (CRP & IL-6) — epigenetic proxies for two key inflammatory markers linked to chronic disease and accelerated ageing
  • Mortality Risk — a statistical risk assessment based on your epigenetic profile
  • Grip Strength & Gait Speed — epigenetic estimates of physical function markers that correlate with longevity and frailty risk
  • Smoking Impact Score — quantifies the epigenetic footprint of smoking exposure on your ageing
  • Alcohol Impact Score — quantifies the epigenetic effect of alcohol consumption on your ageing
  • Cancer Risk — an epigenetic risk assessment for cancer based on your methylation patterns
Q. Who should consider the Epigenetic Biological Age Test?

This test is suitable for anyone aged 18 and over who wants an objective measurement of how their body is ageing. It is particularly relevant if you:

  • Are proactively focused on longevity and want a science-backed baseline to track the impact of dietary, supplement, and lifestyle changes over time
  • Have a family history of age-related diseases — such as Alzheimer's, cardiovascular disease, or cancer — and want to assess whether your current lifestyle is placing you at elevated or reduced risk
  • Follow an anti-ageing or health-optimisation protocol and want to measure whether your interventions are genuinely reducing your pace of ageing
  • Are a practitioner designing preventive health or longevity programmes for patients and need an objective outcome measure
  • Feel that you are ageing faster than expected — experiencing fatigue, cognitive decline, inflammation, or reduced physical resilience — and want to quantify this at a molecular level
  • Want to understand how specific lifestyle factors such as stress, sleep quality, alcohol consumption, or smoking are impacting your biological clock
Q. What is the difference between biological age and chronological age?

Chronological age is simply the number of years since you were born. Biological age reflects how well your body has handled those years at a cellular level — it measures the cumulative effect of diet, exercise, sleep, stress, toxin exposure, inflammation, and genetic factors on the ageing of your cells and organ systems. Two people born in the same year can have very different biological ages depending on how they have lived. A biological age that is lower than your chronological age suggests that your cells are functioning better than expected for your calendar years, and is associated with a longer, healthier lifespan. A biological age that is higher than your chronological age indicates accelerated ageing and a statistically increased risk of age-related disease. The critical insight from this test is that biological age is not fixed — because approximately 60 per cent of DNA methylation is influenced by modifiable factors, it can be actively reduced through targeted changes to diet, exercise, sleep, and stress management.

Q. What is the Pace of Ageing (DunedinPACE) and how is it different from the biological age score?

Your biological age score (OMICmAge) is a cumulative measure — it reflects the total wear and tear your body has accumulated over your lifetime up to this point. The Pace of Ageing (DunedinPACE) is different: it measures how fast you are ageing right now, expressed as years of biological ageing per calendar year. A DunedinPACE of 1.0 means you are ageing at a normal rate; below 1.0 means you are ageing more slowly than average; above 1.0 means you are ageing faster. This distinction is clinically important because a patient might have a good biological age score overall but be currently ageing at an accelerated rate due to recent stress, illness, or lifestyle changes — or conversely, may have an elevated biological age from past exposures but have slowed their current pace significantly through recent interventions. Tracking DunedinPACE over serial tests is one of the most effective ways to measure whether a longevity protocol is working.

Q. Can I repeat this test to track my progress?

Yes — and this is one of the test's most valuable features. Unlike a fixed genetic DNA test (which only needs to be done once because your DNA sequence does not change), an epigenetic methylation test captures a dynamic, changeable state that responds to your diet, exercise, sleep, stress, supplement intake, and environmental exposures. Repeating the test after implementing lifestyle or nutritional changes allows you and your practitioner to see objectively whether your biological age has decreased, whether your pace of ageing has slowed, and which organ systems have responded most. Many longevity-focused patients repeat the test every 6–12 months to monitor progress and adjust their protocol accordingly.

Q. What are the 11 organ system age scores (SYMPHONYAge)?

The SYMPHONYAge section of the report provides individual biological age estimates for 11 major organ systems. Rather than giving you a single number for your whole body, it breaks ageing down by system — so you can see, for example, whether your cardiovascular system is ageing faster than your immune system, or whether your brain is holding up better than your liver. This organ-level granularity is clinically useful because it allows practitioners to identify which systems are most vulnerable and to prioritise interventions accordingly. If your liver age score is elevated, for example, detoxification support and toxin reduction may be recommended; if your immune system is ageing faster, immune-supportive nutrition and stress management may take priority.

Q. How does this test differ from a standard genetic DNA test?

A standard genetic DNA test — such as the Lifecode GX reports offered by the clinic — reads your fixed DNA sequence to identify inherited gene variants (SNPs) that you were born with and that remain the same throughout your life. These tests reveal genetic predisposition. The Epigenetic Biological Age Test does something fundamentally different: it reads the methylation patterns sitting on top of your DNA, which change over time in response to how you live. DNA methylation acts like a set of dimmer switches, turning genes up or down based on environmental signals — diet, exercise, sleep, stress, toxin exposure, and inflammation all leave measurable methylation footprints. This means the epigenetic test provides a dynamic, repeatable measurement of your current health trajectory rather than a static genetic blueprint, and it can show whether your lifestyle choices are actively speeding up or slowing down your ageing at a molecular level.

Q. Can I order this test from outside the United Kingdom?

Yes — this test ships worldwide via DHL, with free return collection included. For most countries you can simply select your location at checkout. If you are based in Australia, Bahrain, Bermuda, Canada, Hong Kong, India, New Zealand, Oman, Qatar, Saudi Arabia, Singapore, South Africa, the United Arab Emirates, or the USA, you will need to contact the clinic for a shipping quote before ordering. Once collected, your blood spot card should be stored in the fridge and ideally returned on the same day to ensure sample quality.

Q. What other tests complement the Epigenetic Biological Age Test?

The biological age test tells you how fast you are ageing but not necessarily why. Combining it with other tests can help practitioners identify the specific drivers of accelerated ageing and design a targeted intervention:

  • To assess current nutritional and metabolic health — the Full Blood Test covers 48 markers including vitamins, minerals, inflammation, liver and kidney function, thyroid, and hormones (UK only, nurse home visit)
  • To understand your genetic predispositions for methylation and detoxification — the Methylation DNA Test and Detoxification DNA Report from Lifecode GX reveal inherited variants that influence how efficiently your body methylates, detoxifies, and manages oxidative stress
  • If gut health may be contributing to inflammation and accelerated ageing — a GI360 Complete stool test can assess gut microbial balance, inflammation, and permeability
  • If hormonal imbalances are suspected — the DUTCH Complete hormone panel measures cortisol, oestrogen, testosterone, and DHEA metabolites, all of which influence the ageing process
  • If toxic burden may be accelerating cellular ageing — the Toxic Metal Urine Screen or Hair Toxic Metal and Essential Elements Test can identify whether heavy metal exposure is a contributing factor

Retesting biological age after 6–12 months of targeted intervention is the most effective way to confirm that changes are having a measurable impact at the epigenetic level.

The Technical Bit and Additional Information

The laboratory extracts DNA, quantifies the DNA, and identifies methylated sites using bisulfite conversion. The DNA is then fragmented, amplified and resuspended. Illumina BeadChips, with silica beads, bind to specific DNA fragments where a single nucleotide is added. An antibody designed to bind to the stain attaches to that nucleotide. (red stain for the AT nucleotide pair, and green for the GC nucleotide.) Over 850,000 CpG sites (spots where methylation can occur) are scanned. The beta values of these CpGs, are then run through specialist algorithms to sort them and create an age value that reflects those patterns. This then provides the personal age, impact, and risk readouts presented in your report.

TruAge™ is an epigenetic test that starts with an at-home blood collection kit. Once your sample is sent back to our lab, they analyse patterns of DNA Methylation to identify your Biological AgeThe TruAge test looks at biological age measured by methylation, as research has shown it is the #1 most accurate and most predictive measure of future disease occurrence and frailty. As a result the predictive power makes it highly useful, both in research and in individual longevity/health tracking. TruAge Complete provides an objective measurement of ageing, and therefore risk of age-related disease, with over 850,000 CpG loci tested. The extensive epigenetic ageing reports and algorithms provide a comprehensive picture of biological ageing and health insights. A Methyl is a little molecule. A clump of them, called a Methyl Group, can stick onto our DNA, changing how the gene in that spot is expressed. Sometimes they turn a gene on or off entirely, and sometimes they just turn the brightness up or down a little bit. That process is called DNA Methylation. So, DNA Methylation is a type of Epigenetic marker, which changes which genes are being expressed, and how much they’re being expressed. DNA Methylation happens in response to daily habits like diet, exercise, sleep, stress, and much much more. About 40% of your DNA Methylation happens in response to the natural progression of time, or during key developmental stages in your gestation and childhood. However, that means the other 60% is Changeable - you can control it with your choices!
